The average additional contribution rate is a calculated figure that is not related to the additional contribution of an individual health insurance fund. The so-called Schätzerkreis, as a panel of experts, forecasts the amount of the expected expenses of all health insurance funds and the income of the health fund for the following calendar year. On this basis, the Federal Ministry of Health determines the average additional contribution rate. It is announced by November 1 each year and applies to the entire following calendar year. In 2026, the officially set average additional contribution rate will be 1,7 percent.
It applies to:
- People who receive basic income support
- People who are to be trained for gainful employment in youth welfare facilities
- Participants in benefits for participation in working life, as well as in clarifications of professional aptitude or work trials
- Trainees who are being trained in a non-company institution as part of a vocational training contract in accordance with the Vocational Training Act (BBiG)
- Disabled persons who work in institutions, homes or similar establishments with a certain degree of regularity and whose earnings do not exceed EUR 707 (2024) per month, which corresponds to one fifth of the earnings of an employee in similar employment who is fully capable of gainful employment
- Persons subject to compulsory insurance who receive injury benefit, sickness benefit or transitional allowance from a rehabilitation provider while receiving medical rehabilitation benefits
- Persons subject to compulsory insurance and voluntarily insured persons whose membership is maintained during military or civilian service or an aptitude exercise
- Members who receive injury benefit under accident insurance, sickness benefit under the Federal Pension Act or comparable compensation benefits
- Trainees whose salary does not exceed 325 euros per month
- Insured persons who perform a voluntary social or ecological year within the meaning of the Act on the Promotion of Youth Voluntary Service (JFDG) or a Federal Voluntary Service under the Federal Voluntary Service Act (BFDG)

If the aforementioned groups of people have other income subject to contributions, for example pensions or retirement benefits, the individual supplementary contribution rate applies to this income.
Important: The amount of the supplementary contribution is also irrelevant for recipients of unemployment benefit I. The employment agency pays the contribution here regardless of the amount of the supplementary contribution of the health insurance fund.
