Chronically ill people often require lifelong medical treatment. And this is very individual: while some lead a completely normal life and hardly need any help, others are dependent on the support of several doctors, therapists and medical facilities. What counts here is the optimal coordination of all those involved.
BKK Wirtschaft & Finanzen offers many chronically ill customers valuable support by participating in BKK WIRTSCHAFT & FINANZEN MedPlus treatment programs – also known as disease management programs (DMPs).
BKK MedPlus creates the organizational and practical framework to ensure that all the doctors and therapists who are important to you pull together with you and treat you according to the latest medical knowledge. The aim is to sustainably improve medical treatment and care. With this additional offer, we want to provide you and your doctor with comprehensive support. The program is aimed at adults as well as parents of children who suffer from the following illnesses and would like to actively contribute to the success of their treatment:
Participation is very simple, voluntary and of course free of charge. However, as more money is made available to patients within the framework of the programs for legal reasons, participation must be declared in writing.
If you think you are eligible for BKK MedPlus, speak to your doctor and sign the relevant participation form. You will then work out a personal treatment plan, your treatment goals and arrange regular follow-up appointments.
If you have only been insured with BKK W&F for a short time, but have been enrolled in the DMP for some time, please re-enrol in the program with a declaration of participation and initial documentation. Simply inform your practice about your change of health insurance provider. You can find further information at www.bkkmedplus.de.
