BKK MedPlus Chronic Disease Management Programs (DMP)

BKK Wirtschaft & Finanzen offers many chronically ill customers valuable assistance here with participation in BKK MedPlus treatment programs – also known as Disease Management Programs (DMP).

BKK MedPlus creates the organizational and practical framework for all the doctors and therapists important to you to work together with you “on the same page” and treat you according to the latest medical findings. It is intended to improve medical treatment and care in the long term. With this additional offer, we want to provide comprehensive support for you and your doctor. The program is aimed at adults, but also parents of children, who suffer from the following diseases and want to actively contribute to the success of their treatment:

• Diabetes mellitus type 1
• Diabetes mellitus type 2
• Breast cancer
• coronary heart disease (CHD)
• Asthma
• COPD (chronic obstructive pulmonary disease)
• Osteoporosis (in Berlin, Bremen, Hamburg, Hesse, North Rhine-Westphalia, Mecklenburg-Western Pomerania, Schleswig Holstein and Westphalia-Lippe)
• Rheumatoid arthritis (in Schleswig Holstein)

Participation is very simple, voluntary and of course free of charge. However, as more money is made available to patients within the framework of the programs for legal reasons, participation must be declared in writing.

If you believe that you are eligible to participate in BKK MedPlus, please speak to your doctor’s office and sign the corresponding participation form there. For the further course, you will jointly develop a personal treatment plan, your treatment goals and arrange regular follow-up appointments.

If you have only been insured with the BKK W&F for a short time, but have been enrolled in the Disease Management Program (DMP) for longer, please re-enroll in the program with a declaration of participation and initial documentation.
