Your individual need for assistance will be determined as part of a care assessment so that you receive the necessary support.
Implementation
Assessors from the MD conduct the care assessment. The assessors are specially trained nursing professionals or physicians who come to your home—this can also be a retirement or nursing home. The MD will suggest an appointment for the home visit in advance, either in writing or by telephone.
Procedure
The MD’s expert will come to you to get an idea of your personal care situation. Describe to him what limitations you have and what difficulties you have in everyday life. This information will help the expert to correctly assess your situation. The home visit can take up to one hour.
Preparation
Please ask the person who primarily cares for you or who is particularly familiar with your situation to be present during the home visit. If you have a legal guardian, please inform them about the home visit. You should have all documents and information for the assessor ready. The MD’s checklist will help you with this. Please have your ID ready for the visit.
Result
The MD summarizes the results in an expert opinion. It states the care level you will be classified in. The MD forwards the report together with its recommendations on your need for aids and further therapies to us. You will then receive a notification from us about the benefits available to you, as well as a copy of the report. As a rule, it takes up to 25 working days from the application to the notification.
